Co-op Survey Forms

At the completion of each co-op term you will be responsible for ensuring that all of the co-op evaluation surveys are completed and returned to your co-op advisor.  

• Student's Evaluation of Co-op Work Experience survey form will be emailed to you approximately 3 weeks prior to the end of your co-op semester.  Follow the email instructions to complete and submit to your advisor.
   
• Employer's Evaluation of Co-op Student survey form will be emailed to you in a separate email at the same time as the student survey. This email is to be forwarded to your co-op supervisor.  Instructions to complete and submit is in the e-mail.
